FMEA Flow

Hierarchical failure mode & effects analysis made simple

Stay focused. Build a complete FMEA without getting lost.

Create assets, break them into child systems, identify failure modes, score risk, and turn findings into a clear action plan. This app keeps everything hierarchical, mobile-friendly, and easy to review.

Quick-start prompts

Helpful questions a good FMEA should answer.

Risk legend

Simple rating logic to keep scoring consistent.

Severity: How bad is the effect if failure happens?
Occurrence: How likely is the cause or failure mode to happen?
Detection: How likely are you to detect it before impact?
RPN: Severity Γ— Occurrence Γ— Detection
Low < 80
Medium 80–199
High 200+

FMEA

Build a hierarchical FMEA from assets to functions, score failure risks, prioritize actions, and export a review-ready report.

What this tool does

FMEA Flow organizes a risk review as an asset, subsystem, component, and function hierarchy. You can attach failure modes to any node, calculate RPN, review all rows in one table, and turn open actions into a prioritized completion plan.

Who this is for

How to use this tool

  1. Set the project title, owner, site, date, scope, and success criteria
  2. Use Structure to build the hierarchy and add or select a node
  3. Record each failure mode’s effect, cause, controls, action, owner, and due date
  4. Score Severity, Occurrence, and Detection from 1–10 and review the RPN
  5. Use Analysis and Actions to prioritize work, then save JSON or export PDF

Inputs

Inputs include project context, hierarchical node names and functions, optional codes and notes, failure effects and causes, controls, recommended actions, owners, due dates, and 1–10 risk scores. A sample project can be loaded, and saved JSON can be imported.

Outputs

FMEA Flow shows asset and failure-mode counts, focus and completion scores, a hierarchy, selected-node analysis, a classic all-rows table, risk labels, an action plan, and a completion tracker. It calculates RPN as Severity Γ— Occurrence Γ— Detection; the displayed bands are Low below 80, Medium from 80–199, and High at 200 or above.

Frequently asked questions

How is the priority score used?

The tool combines severity, occurrence, and detection inputs into a risk-priority value to help rank review items; it is a planning score, not a validated failure probability.

Can I export an FMEA report?

Yes. Export the review-ready report using the page’s available export controls. Confirm the rendered report before distributing it.

Does the hierarchy prove a cause?

No. It organizes assets, functions, failure modes, effects, causes, and actions for team review; subject-matter validation is still required.
