What this tool does

Nexus FMEA helps you list system components and their failure modes, score each one for severity, occurrence, and detection, and calculate a Risk Priority Number (RPN). It includes an equipment-template library and a rating guide for consistent reviews.

How to use this tool

  1. Open the Editor tab and name the project
  2. Add a component or choose one of the equipment presets
  3. Describe each failure mode, effect, cause, control, and recommended action
  4. Set Severity, Occurrence, and Detection from 1–10 to calculate RPN
  5. Review the Risk Tree or Rating Guide, then save JSON or export a PDF report

Inputs

Inputs include project and component names, intended functions, failure modes, effects, causes, current controls, recommended actions, owner/date fields, and 1–10 Severity, Occurrence, and Detection scores. Existing JSON projects can be loaded.

Outputs

The app calculates RPN as Severity Γ— Occurrence Γ— Detection, color-codes risk, displays component and failure-mode summaries, and produces JSON and PDF exports. Its thresholds label RPN values below 50 as acceptable, 50–99 as warning, and 100 or higher as critical.
